NH Smiles & Medicaid referrals

Canobie Dental participates with NH Smiles Adult and Children's Dental Benefits and accepts new patients at our Newport office.

Healthcare and community organizations may refer eligible New Hampshire Medicaid patients who need a dental home.

Coordination with the referring provider

A referral should make care easier for the patient.

If a patient is referred for a specific service, we can communicate the scope of treatment and return or co-manage the patient as appropriate.

If the referral is for a comprehensive dental home, we can assume ongoing general dental care while coordinating with the patient's other healthcare providers when needed.

For general professional questions that do not contain patient information, contact Canobie Dental directly.

Patient records, radiographs, medical information, and other protected information should be sent only through the secure referral process provided by our office.

Do not send patient information through an ordinary public website form or unsecured email.
