HIPAA Notice of Privacy Practices

Effective August 26, 2026

THIS NOTICE DESCRIBES HOW HEALTH INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Your Rights

You may:

  • inspect or get an electronic or paper copy of your health record;
  • ask us to correct information you believe is wrong or incomplete;
  • ask us to contact you in a specific way or at a specific place;
  • ask us to limit certain uses or disclosures. If you pay in full out of pocket for a service, you may ask us not to share information about that service with your health plan for payment or health care operations, and we will comply unless the law requires otherwise;
  • request an accounting of certain disclosures made during the prior six years;
  • get a paper copy of this notice;
  • have a legally authorized personal representative act for you; and
  • complain if you believe your privacy rights were violated.

Your Choices

You may tell us your preferences about sharing information with family, friends, caregivers, or others involved in your care or payment, and about disaster-relief disclosures.

We will obtain your written authorization for uses or disclosures not described in this notice when authorization is required, including most uses for marketing, the sale of protected health information, and most uses of psychotherapy notes. You may revoke an authorization in writing, except to the extent we have already acted on it.

How We May Use and Share Your Information

We may use or disclose your health information as permitted or required by law, including to:

  • treat you and coordinate your care;
  • bill and obtain payment;
  • operate our practice, improve care, train staff, conduct quality and compliance activities, and communicate with you;
  • address public-health and safety matters;
  • conduct permitted research;
  • comply with federal or state law;
  • respond to health-oversight, workers' compensation, law-enforcement, government, court, subpoena, or other lawful requests;
  • work with coroners, medical examiners, funeral directors, or organ-procurement organizations; and
  • share relevant information with people involved in your care or payment when permitted.

New Hampshire and other laws may provide additional confidentiality protections. When a stricter law applies, we follow the stricter rule.

If we maintain substance-use-disorder patient records protected by 42 CFR Part 2, we will not use or disclose those records in civil, criminal, administrative, or legislative proceedings against you without your written consent or a qualifying court order and subpoena, as required by law.

Our Responsibilities

We are required by law to protect the privacy and security of your protected health information, follow the notice currently in effect, and notify you when required if a breach may have compromised your information.

We will not use or disclose your information other than as described here unless you authorize us in writing or the law otherwise permits or requires it.

We may change this notice. A revised notice may apply to all health information we maintain and will be available at our office and on our website.
